Optimizing Optometric Scope: A Safe, Evidence-Based, and Essential Step Forward to Improve Access to Care

The Canadian Association of Optometrists (CAO) is working closely with governments across Canada to optimize optometrists’ scope of practice as an essential, evidence-based response to growing eye health needs.

With aging populations, more chronic eye disease, and increasing strain on specialist capacity, provinces are looking to proven, safe solutions that improve patient access to timely care.

In both Alberta and Ontario, regulators are reviewing safe additions to optometrists’ authorized procedures, specifically in-office, non-sedated interventions such as YAG capsulotomy for secondary cataracts, selective laser trabeculoplasty (SLT) for glaucoma management, and removal of benign eyelid lesions. These procedures are already included in North American optometry curricula and are performed safely by optometrists in multiple U.S. states and in the U.K.

“As the front line of vision health and eye care, optometrists are highly trained, regulated professionals who already provide comprehensive diagnosis and management of ocular disease,” said Dr. Allison Scott, President of CAO. “Optimizing scope is about ensuring patients can receive safe, effective care sooner. Optometrists and ophthalmologists both play essential roles. Optimized scope simply allows each profession to work to its strengths while keeping patient safety at the centre.”

Canada faces pressing access challenges: months-long specialist wait times, rapid increases in glaucoma, diabetic retinopathy, and age-related macular degeneration, and persistent gaps in rural and northern communities.

With 7,135 optometrists across Canada – compared to 1,382 ophthalmologists – allowing optometrists to provide routine in-office procedures will reduce delays, support earlier intervention, and allow ophthalmologists to focus on complex surgeries that require their expertise.

Optometrists are not seeking authority to perform intraocular surgeries such as cataract extraction, retinal operations, or corneal transplants. The proposed updates involve only the safe, office-based procedures aligned with their training and international standards.

Optometrists complete seven to eight years of post-secondary education, including a Doctor of Optometry degree with extensive training in ocular disease, pharmacology, laser concepts, systemic disease, and supervised clinical practice over multiple years. Advances in the education and training of optometrists have outpaced their existing scope of practice. Doctors of Optometry are already educated and clinically trained to practice at a higher level than legislated.

To maintain licensure, optometrists must also complete ongoing continuing education, ensuring that their clinical skills remain current, evidence-based, and aligned with evolving standards of care.

Their safety record is well established:

  • A review of 146,403 laser procedures performed by optometrists in the U.S. found a complication rate of 0.001%1.
  • A 2023 study in Optometry & Vision Science found YAG capsulotomies performed by optometrists to be safe and effective, with 99% patient-reported vision improvement, 95% objective improvement, and no significant adverse events2.

“These procedures are well within optometric training and are performed safely worldwide,” said Dr. Scott. “Our profession’s record speaks for itself: optometrists put patient safety first in every clinical decision we make, upholding the highest clinical standards, and acting in their best interests.”

CAO will continue working with governments, regulators, and vision care partners to ensure Canadians – especially those in underserved communities – have access to timely, optimal eye care without compromising safety.

Contact Lens Institute Releases New Report on Generational Eye Care Trends

The Contact Lens Institute (CLI) has published a new research report, Shifting Focus: Generational Perspectives of Contact Lenses & Eye Care, offering fresh insights into how Gen Z, Millennials, and Gen X evaluate eye care providers, practice experiences, and vision correction products. The report is available as a free downloadable PDF from the CLI’s See Tomorrow resource hub.

The publication presents data, infographics, and analyses on generational differences in contact lens and glasses use, purchasing motivations, perceptions of value, service expectations, and loyalty drivers. Each section is paired with practical guidance from CLI Visionaries—optometrists and opticians who outline ways to translate findings into meaningful actions for practices and teams.

“What we discovered is at times surprising, yet essential as eye care business owners and managers prepare for the decades ahead,” said Stan Rogaski, executive director of CLI. “By better connecting with people in the exam lane or optical floor, individuals and teams are more likely to strengthen patient relationships and long-term success.”

Key Findings Include:

  • Contact lens wearers are significantly more loyal to their eye care providers compared to glasses wearers, particularly among Gen X (77% vs. 54%).
  • Millennials report the highest rate of contact lens use (43%), followed by Gen Z (35%), highlighting opportunities to further engage younger patients.
  • Across all generations, at least 70% of patients want their eye care team to clearly explain the value and lifestyle benefits of vision correction recommendations.
  • Younger consumers—Gen Z and Millennials—place strong emphasis on authenticity, personalization, and social/environmental responsibility in their eye care experiences.
  • Expectations for non-traditional hours differ sharply: roughly half of patients ages 15–44 believe practices should be available for evening care or inquiries.
  • Despite broad access to online health information, patients still view their eye care team as their most trusted experts, far surpassing digital or AI sources.

Shifting Focus is part of the CLI’s See Tomorrow initiative, which aims to deliver data-driven insights that support soft contact lens prescribing, healthy wear and care, and practice growth. The findings are based on a Summer 2025 survey of 1,308 vision-corrected consumers in the U.S. and Canada.

Zyloware Announces Retirement of Co-CEO Chris Shyer

Zyloware Eyewear has announced that Christopher D. Shyer, the company’s co-CEO and president, will retire from his executive role effective November 25, 2025. Shyer will remain a co-owner and continue to support the company’s direction and leadership team.

Shyer has been part of the family business since 1989, guiding Zyloware through major industry shifts, economic disruptions, evolving retail landscapes, and global challenges ranging from the expansion of vision insurance to the COVID-19 pandemic. He also played a key role in leading Zyloware through its 100th anniversary and in securing the investment from CenterGate Capital that enabled the acquisition of Kenmark Eyewear—an expansion the company says reinforces Zyloware’s position as the longest-running family-owned independent optical company in the U.S.

“Zyloware’s 100+ year old American legacy puts it in league with Disney, Coca-Cola, and Kellogg,” Shyer said, noting his ongoing commitment as an owner and supporter of the next generation of company leaders.

The company emphasized Shyer’s reputation for building long-standing relationships across the industry and fostering a culture rooted in collaboration and trust. His decision to step down, Zyloware stated, comes “with great reflection and deep appreciation for the company and the industry he has served so wholeheartedly.”

Shyer’s family has deep roots in the optical sector, including leadership roles with the Optical Manufacturing Association and The Vision Council. His own contributions include chairing key industry committees and co-creating the “Check Yearly, See Clearly” public health campaign. He also once consulted on an episode of Seinfeld involving a memorable purchase of women’s glasses by the character George Costanza.

Before joining the family business, Shyer earned his MBA from Columbia University and worked at Lever Brothers. His early years at Zyloware included training in the company’s New York City factory, where he learned the production process and design principles that shaped his approach to engineering fit, quality, and value for retailers and consumers. Throughout his career, he expanded the company’s licensing portfolio and built internal expertise in design, quality assurance, marketing, and analytics—helping Zyloware achieve one of the industry’s lowest back-order rates.

Shyer has also been recognized as an early LGBT leader in the eyewear business and is co-author of the award-winning memoir Not Like Other Boys. His philanthropic work spans organizations including GLSEN, The Acting Company, Public Health Solutions, and VisionSpring. He plans to continue these pursuits and will teach master classes in industrial design in the U.S. and abroad.

“My cousin’s dedication, vision, and tireless work ethic will be deeply missed day-to-day in the business,” said Jamie Shyer, co-CEO/COO. “But his continued co-ownership will inspire us as we carry forward the mission and values he has spent his life building.”

Zyloware expressed gratitude for Shyer’s decades of leadership and noted it looks forward to his continued involvement as the company enters its next chapter.

Quebec’s Bill 2 and Its Impact on Access to Vision Care

The Canadian Ophthalmological Society (COS) expresses deep concern over the passage of Bill 2 by the Government of Quebec. This legislation, which unilaterally concludes negotiations with the province’s medical federations, risks destabilizing a health system already under significant strain and may negatively affect access to essential eye care for Quebecers.

Ophthalmologists provide specialized, often urgent care that protects and restores sight. The success of this work depends on collaboration, respect, and a stable environment that allows physicians to focus fully on their patients. Bill 2 was introduced without sufficient dialogue, creating uncertainty and undermining trust between physicians and the government. Such uncertainty can discourage professionals whose expertise is vital to maintaining quality care across the province.

Even in challenging circumstances, physicians remain deeply committed to caring for their patients. However, sustaining that commitment requires conditions that support and value their work. Measures that weaken morale or drive specialists from practice risk further straining access to timely treatments and increasing wait times for essential care.

The COS respectfully urges the Government of Quebec to engage in renewed and constructive dialogue with the medical community. Collaboration, not legislation, is the path to sustainable solutions that serve both patients and health professionals.

Our Society stands with Quebec’s ophthalmologists and all health professionals who share a commitment to patient care. Vision is one of our most precious senses. Protecting it requires a health system grounded in partnership, trust, and respect.
